Initial Comments:
Based on the findings of an onsite unannounced home health agency Medicare recertification survey conducted February 23-24, 2021, America's Home Health Services was found not to be in compliance with the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The agency ceased to operate as a Home Health Agency. The agency reported via email on February 23, 2021, that there were no patients on census since January 31, 2019. There is no evidence that the agency submitted an Outcome and Assessment Information Set, (OASIS), data since 10/01/2019. The OASIS OUTCOME report revealed the agency submitted OASIS data from a single OASIS discharge assessment sometime between 01/2017 and 12/2018 as indicated under " Discharge to Community." The Final Validation Report could not be generated due to no evidence that the agency submitted OASIS data for the previous year and current fiscal year, October 1, 2020.
The agency is not in compliance with the definition of a Medicare Home Health Agency. For purposes of Medicare participation, a "home health agency" is defined at section 1861(o) of the Social Security Act to be: "a public agency or private organization ... which -- (I) is primarily engaged in providing skilled nursing services and other therapeutic services." The PA Department of Health is sending a recommendation to Centers for Medicare & Medicaid Services that America ' s Home Health Services located at 9622 Bustleton Avenue, Suite 2, Philadelphia, PA 19115, be involuntarily terminated as a provider of services.

Based on the findings of an attempted onsite unannounced state relicensure survey completed February 23, 2021, and offsite on February 24, 2021, America's Home Health Services was found NOT to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart G, Chapter 601, Home Health Care Agencies. The agency has ceased to operate as a home health agency and is not engaged in providing skilled nursing services and other therapeutic services. As per facility documentation received on February 23, 2021,the last patient was discharged on January 31, 2019.
